The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together we create personalized plans that improve outcomes and ease the transition to home.</p>
<h3>Job Summary</h3>
<p>We are hiring dynamic, passionate <b>Speech Language Pathologists</b> (SLPs) who want to be part of transformational change in home and community care by providing care in a truly client‑centered approach. You will serve a roster of clients in the community, conduct joint care planning focused on client‑centered goals, and collaborate with an interdisciplinary team to reach those goals.</p>

<h3>Job Responsibilities</h3>
<ul>
<li>Conduct assessments.</li>
<li>Formulate goals and recommendations.</li>
<li>Plan and implement client‑centered, goal‑directed treatment programs.</li>
</ul>
<h3>Requirements</h3>
<ul>
<li>Graduate of an accredited Speech Language Pathology program and in good standing with the College of Speech Language Pathologists of Ontario.</li>
<li>Recent experience managing patients in acute/rehab or community‑based rehabilitation programs is preferred.</li>
<li>Reliable, flexible, self‑directed professional able to work effectively with rehab assistants and an interdisciplinary team.</li>
<li>Sound knowledge of community resources is an asset.</li>
<li>Effective interpersonal, organizational and communication skills.</li>
<li>Positive work and attendance record is essential.</li>
<li>Ability to adapt to new computer applications.</li>
<li>Hybrid work approach: remote and in‑service with clients in their settings.</li>
<li>Valid driver’s license, vehicle and ability to travel.</li>

<h3>Compensation and Additional Information</h3>
