12 – $41. 46 per hour We are currently seeking a Registered Nurse – Outreach (Relief).

As part of the Outreach Team, you will provide services to the homeless population in Peel Region, in particular, identifying people who are homeless or at risk of homelessness and engaging them in services that support their challenges with serious mental illness or mental illness and substance abuse (concurrent disorders). This is a relief position with 12 hour shifts mostly in the evenings, weekends and overnights.

The incumbent must be comfortable driving the Outreach Van across the Region of Peel. What you will do Work autonomously and utilize clinical judgment in providing comprehensive nursing care.

In collaboration with the Physician and/or Nurse Practitioner, coordinate, schedule, administer medical assessments, medication, referrals for health, social and other medical services. Participate in the rotational 12-hour Outreach schedule which involves community street outreach support, working on the Mobile Health Clinic, and answering the 24/7 Street Help Line.

Collaborate with families and Community Service Providers to develop care plans for clients who are street homeless and/or residing in shelter or hotel. Conduct on-site Community Nursing Clinics at key drop-in locations in the Peel Region.

It’s very powerful and satisfying. ” Registered Nurse What you need to join our team Current registration as a Registered Nurse with the College of Nurses of Ontario in good standing 2 to 3 years demonstrated Nursing experience in a background in Primary, Urgent and/or Critical Care Experience/interest in working with communities facing barriers to accessing health care due to poverty, social isolation, homelessness, mental health and/or substance use challenges Primary care health assessments and education Understanding of the social determinants of health as they impact persons experiencing homelessness Comfort working with an electronic charting system Ability to work 12-hour shifts covering evenings, weekends and overnights Valid Ontario driver’s license, relevant vehicle insurance and access to a reliable vehicle Proof of administered full-series Health Canada approved COVID-19 vaccination by prior to start date.

If unable to be vaccinated for reasons relating to protected grounds under the Code, completed medical or religious exemption must be provided.
