We support clients of all ages to live safely and well in their homes through compassionate, high-quality care.</p><p>We are currently seeking an experienced <strong>Nurse Practitioner (NP)</strong> to provide <strong>virtual clinical oversight and interim primary care support</strong> for a defined community care area. Many encounters are handled <strong>virtually</strong>, with <strong>in-person visits required only when clinically necessary and pre-approved</strong>.</p><p>Each Nurse Practitioner is assigned <strong>one service area</strong>, with coverage structured to be focused, predictable, and collaborative.</p><p>Each Nurse Practitioner supports a <strong>small cluster of neighboring communities</strong> within Ontario Health at Home’s <strong>Central service area.</strong></p><p>Examples include:</p><ul><li> York Region & North Toronto corridors </li></ul><p>The role is <strong>virtual-first</strong>, with in-person visits required only when clinically appropriate and <strong>within the assigned cluster</strong>.</p><p>This role is <strong>not clinic-based</strong> and <strong>not daily home visiting</strong>.</p><p><strong>Requirements</strong></p>Interim Primary Care Responsibility<p>Some clients supported through this role may be <strong>temporarily without a family physician</strong>.</p><p>In these situations, the Nurse Practitioner provides <strong>interim, time-limited primary care oversight</strong>, including assessment, medication management, and clinical decision-making, <strong>until long-term primary care attachment is secured</strong>.</p><p>This is <strong>not an open-ended or indefinite rostered role</strong>.<br /> Care is delivered within a <strong>shared, interdisciplinary model</strong> and is tied to the duration of the home-care episode.</p><p></p>Key Responsibilities<ul><li> Provide virtual clinical consultations (video, phone, and chart review) </li><li> Deliver interim primary care oversight for unattached clients when required </li><li> Review changes in condition and recommend next steps </li><li> Support care planning and escalation decisions </li><li> Collaborate closely with nursing and therapy teams </li><li> Conduct in-person visits only when clinically necessary and pre-approved</li></ul><p></p>Work Structure<ul><li> On-call, consultative model </li><li> Virtual-first delivery </li><li> Assigned one defined service area </li><li> Flexible scheduling by mutual agreement </li><li> In-person visits are infrequent and purposeful </li></ul><p><strong>Benefits</strong></p>Compensation & Mileage<p>We offer <strong>competitive, consult-based compensation</strong> that reflects senior clinical expertise:</p><ul><li><strong>$110–$140 per virtual consult</strong> (30–45 minutes) </li><li><strong>$160–$200 per in-person visit</strong>, when required </li><li><strong>$75/hour for any other duties (admin, etc)</strong></li><li> Mileage reimbursed at competitive rate</li></ul><br />Qualifications<ul><li> Registered Nurse Practitioner in good standing with the CNO </li><li> Comfortable working at full NP scope </li><li> Strong experience with complex or medically fragile clients </li><li> Confident and thoughtful clinical decision-maker </li><li> Comfortable delivering care virtually </li><li> Values flexibility, autonomy, and collaborative practice </li></ul><p></p>Why Work With The Care Company<p>At The Care Company, our people are the heart of everything we do.
