Under the direct supervision of the Clinical Lead, Nursing and the Program Manager Concierge Medicine, the Concierge Medicine Nurse is an important member of the Concierge Medicine healthcare team. The Concierge Medicine Program provides patients with ongoing care and access to a dedicated team 24 hours a day, 7 days a week.


Education, Knowledge, Skills and Abilities
Excellent patient assessment, education and triaging skills.
Exceptional verbal and written communication skills.
Knowledge of Ontario immunization guidelines.
Working experience with EMR software (preferably EPIC) and web-based applications.

Ability to work independently as well as collaboratively in a patient-focused care environment within a multidisciplinary team.

Phlebotomy knowledge is an asset.

Required Length and Type of Experience
At least 3 years of recent acute care experience, preferably in a hospital setting, as a Registered Nurse/Registered Practical Nurse.

Required Licensure, Certification or Registry
Current registration as a Registered Nurse/Registered Practical Nurse in good standing with the College of Nurses of Ontario.
Must have current BCLS certification.

Provides professional nursing care to a specific patient population utilizing the nursing process.

Patient assessment includes measuring vital signs. Communicates patient concerns to physician and/or appropriate resources.
Performs a variety of procedures such as ear irrigation, suture/staple removal,nasopharyngeal specimen sampling, intramuscular injections, as well as assists physicians with Pap smears.
Administers and records immunizations. Educates patients regarding possible side effects.
Performs or assists with various procedures as competency and training permit (e.g.audiometry and visual acuity testing, spirometry, phlebotomy, and Specialist Clinics procedures).
Triages patient phone calls and emails appropriately. Assesses patients’ health needs andschedules patient consults as required.

Communicates patient concerns to physician and/or appropriate resources.
Manages prescription renewals when required (pro-active prescription management).
Acts as a case manager, working closely with the physician to establish a follow-up plan for patients, ensuring appointments are made and following up on all results, referrals and consult letters.
Discusses test results and responds to questions pertaining to health promotion and disease prevention. Adapts education to meet patients’ needs.
Acts as a patient advocate and liaison between patient, family and members of the healthcare team. Assesses home support network of patients, utilizes resources effectively and efficiently promoting quality patient care.
Actively maintains infection control as well as a safe working environment.
Maintains accurate, concise and confidential electronic medical record documentation (i.e.EPIC) pertaining to patient history and encounter data.
Demonstrates leadership skills in the coordination and provision of healthcare. Takes responsibility for identification of own professional growth and development needs.

Meets all departmental/organizational education requirements.
Interacts with patients, physicians, staff and other caregivers in a caring, compassionate and supportive way. Resolves difficult conversations or situations in a mature and professional manner with input from others as needed.
Available to participate in an after-hours on-call coverage schedule with a Concierge Physician that includes weekends and holidays
