Case Management Nurse

Job Description

The Nurse Case Manager is the clinical backbone of a specialty patient program—combining patient education, benefit navigation (as applicable), scheduling/coordination, and close communication with prescribers, pharmacies, and other stakeholders. You’ll help patients start therapy successfully, maintain continuity of care, and feel supported throughout their treatment journey.


What You’ll Do (Key Responsibilities)

  • Enroll & welcome patients: Complete onboarding and welcome calls following defined scripts; ensure accurate capture of clinical/admin details and consent.
  • Coordinate care: Schedule diagnostics/appointments; follow up on expired or missing test results; verify next appointments; track missing post‑visit reports (where applicable).
  • Educate & support: Provide therapy‑related education to patients and HCPs; offer and/or deliver product training (as program permits).
  • Reimbursement support: Where program‑specific, liaise with payors to determine coverage options; advocate and follow up until a decision is received.
  • Stakeholder liaison: Maintain transparent communication with physicians, nurses, PSP teams, pharmacy partners, and manufacturer field teams to resolve barriers.
  • Documentation & data integrity: Maintain accurate patient records; ensure data integrity to meet program reporting requirements.
  • Compliance & safety: Identify and report AEs to appropriate channels; complete product complaint reports (as applicable); adhere to privacy regulations and Quality Management System practices.
  • Operational excellence: Complete time/expense reports (if required), participate in training and continuous improvement, and support program KPIs. Complete other tasks as requested.


What You Bring (Requirements)

  • Active nursing registration (RN or RPN/LPN) in good standing with a Canadian provincial regulatory body.
  • Experience: Minimum 2 years in Patient Support Programs or equivalent clinical coordination/education roles.
  • Clinical/technical skills: Strong clinical judgment, problem‑solving, conflict resolution, and patient‑centric communication; proficient with Excel/Word/Outlook, tablets, web portals, and virtual tools.
  • Reimbursement literacy: Understanding of public/private coverage criteria and SA/PA processes (asset, program‑dependent).
  • Documentation excellence: Accurate data collection, data integrity mindset, and comfort working in a paperless environment.
  • Privacy & compliance: Familiar with PIPEDA and PV/AE reporting expectations; able to work autonomously in a remote/virtual team environment when required; reliable high‑speed internet.


Education & Credentials

  • Graduate of a recognized Registered Nursing program or Registered/Licensed Practical Nursing program (current registration required).


Nice to Have

  • Bilingual (English/French)—may be required depending on program/region.
  • Therapeutic‑area experience (e.g., oncology, immunology, rare disease).
