Background: Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

Position Summary:

FHC recently received funding from the City of Toronto to create a more inclusive, accessible, and capacity building system through community-based, population-focused engagement, while creating effective safety prevention activities and enhanced Network functions. The focus is on supporting the transition from agency-led to resident/grassroot safety networks across the City of Toronto.



This unionized position will operate through central district and will oversee around 6-8 safety networks in each district. The position will be responsible for supporting resident-led safety networks, identify annual priorities, provide access to space, organize events and trainings and sustain adequate membership. The target audience in this Project are residents, grassroots, and community members who are involved in community related activities that address safety issues from a multi-layered lens.

Responsibilities include:

  • Develop, coordinate, implement, deliver, and report on the Enhanced Safety Network goals
  • Recruit, train and supervise up to 4 Enhanced Safety Network Ambassadors, in collaboration with agency partners, to oversee daily outreach initiatives with the goal of improving awareness, education and navigating systems (refer to ambassador role description for more information)
  • Identify strategic approaches to increase uptake of service coordination for intended individuals and communities within the assigned district
  • Act as the primary liaison for FHC, with the City of Communicate with district City of Toronto Community Development Officers to consult and support the informing of district plans and trends/themes
  • Ability to provide and direct Ambassadors’ outreach and service in “non-traditional” and culturally appropriate, community-based ways that are responsive to the needs of gang involved and/or families and marginalized communities impacted by violence.
  • Ability to provide and direct Ambassadors’ to culturally appropriate prevention and intervention services to safety networks, resident leaders who support local community members through service coordination, youth violence education, cross community conflict and training supports, cultural and community education, mental health supports
  • To liaise, consult and collaborate with key stakeholders in the community: City services, Toronto Community Housing Corporation, Provincial Youth Outreach Workers, mental health and employment focused agencies and other service providers as required regarding the needs of gang involved families. Support Ambassadors with key stakeholder management.
  • Facilitate Safety Network planning, identify priorities, participate in Community of Practice and team meetings, take minutes and keep partners updated on current trends and upcoming opportunities that further enhance capacity building.
  • Amplify City messaging about the Safety Network and coordinate the development of content such as public service announcements, social media posts, animations, and videos.
  • Arrange opportunities for community health and social services education (agency site- visits/tours/workshops) for the Safety Network in each district.
  • Ensure ongoing “peer to peer” community of practice for Safety Network Ambassadors to allow information sharing, decompression, and issues escalation (this will be hosted monthly with other members of Safety Network teams)
  • Develop common evaluation tools to track service coordination (3 touch points) and implementation measures to ensure that programs are effective in increasing awareness, education and navigating systems for Safety Network confidence and functioning.
  • Research, evaluate and identify potential training for providers appropriate to the deliverables of the Safety Network Project.
  • Identify training and development needs within the organization through consultation and collaboration with internal and external resources.
  • Coordinate on multidisciplinary project teams, maintain professional relationships with other organizations and key partners.
  • Deal with confidential and sensitive information regarding operations, assets, and resources within the Safety Network Project in a professional manner.
  • Ensures high quality customer service through communications strategies including training, education material, and best practices within the Project.
  • Compile and submit monthly reports to document engagement approaches, initial contact from key partners, emerging governance or operational issues contributing to pilot successes and
  • Compile quarterly/monthly Safety Network Project status reports, work plan updates on activities, highlights, and budgets.
  • Provides input into and administers assigned budget, ensuring that expenditures are controlled and maintained within approved budget limitations.


Qualifications:

  1. Lived experience and/ or experience working with Indigenous Peoples, Black Peoples and Newcomer communities, and/or organizations is an asset


2. Lived experience and/ or experience working in marginalized communities with most vulnerable residents and/ or families impacted by gun violence, human trafficking, food scarcity, unemployment, mental health and cross community conflict.


  1. Experience working in different districts of the city is an
  2. Knowledge of and sensitivity to the life experiences and needs of families and marginalized communities impacted by violence or gang involvement.
  3. Post-secondary degree or Community College diploma in social services or a field relevant to youth development, family development, social justice systems, or equivalent combination of education and experience. Minimum of two-three years’ experience working with most vulnerable and/ or gang/violence involved families and/ or marginalized communities.
  4. Considerable experience identifying community or safety network needs and conducting research to develop strategies and best practices.
  5. Experience with planning, implementing and assess learning processes in the lens of community
  6. Experience designing and delivering community based projects/programs.
  7. Considerable experience using Microsoft Office applications (MS Word, Excel and PowerPoint) to produce reports and presentations.
  8. Experience with safety networks, practices, training, and evaluation
  9. Fundamental knowledge of community development and coordination of services and
  10. Well-developed written and verbal communication
  11. Fundamental knowledge of organizational effectiveness principles and how they relate to overall program outcomes
  12. Ability to learn new reporting systems (e.g. online contact referral process and electronic tracking systems) to track and initial contacts into the pilot.
  13. Vaccines (COVID-19 and others) are a requirement of the job pursuant to the Ontario Human Rights Code
  14. Experience working from an anti-racist/ anti-oppression framework (ARAO)
  15. Valid Police Records Check
